We're looking for a passionate, outgoing, and results-driven Account Executive to help us expand our impact throughout the Phoenix area. What You'll Be Doing As the face of Total Care Connections in the community, you'll play a key role in growing our referral network and helping families find the care they need.

Your Responsibilities Include

• Building and nurturing relationships with referral partners, including hospitals, skilled nursing facilities, physicians, case managers, the VA, Banner, UnitedHealthcare, and private-pay sources
• Conducting client assessments and meeting with families to understand their care needs
• Developing personalized care plans that set clients up for success
• Collaborating with Care Managers and Operations teams to ensure seamless service delivery
• Serving as a trusted resource for clients, families, and referral partners throughout the onboarding process
• Representing Total Care Connections at networking events and throughout the Phoenix healthcare community

• Has a passion for helping seniors, veterans, and adults with disabilities

• Can balance business growth with genuine compassion and care

Preferred Experience

• Home care, healthcare, hospice, senior living, or community-based services experience
• Knowledge of Arizona Medicaid HCBS programs
• Experience working with referral sources such as the VA, Banner, UnitedHealthcare, hospitals, SNFs, and community partners
• Private-pay business development experience
• Client assessment and care planning experience

Additional Qualifications

• Bilingual (English/Spanish)
• Existing referral relationships within the Phoenix healthcare community
• Experience working with seniors and disabled populations
