What they need

ACT CASE MANAGER

FMRS Health Systems, Inc. is seeking a dedicated and motivated Case Manager to join our Assertive Community Treatment (ACT) Team.

The ACT Team provides intensive, community-based behavioral health services to adults with serious and persistent mental illness. This position is ideal for someone who enjoys working directly with individuals in the community, problem-solving, coordinating resources, and being part of a multidisciplinary treatment team.

Position Responsibilities

The ACT Case Manager works collaboratively with ACT team members to help consumers maintain stability, increase independence, and successfully live in the community. Responsibilities include:

  • Providing intensive case management and supportive services in consumers' homes and other community settings.
  • Assessing consumer needs, strengths, barriers, and available supports.
  • Participating in the development, implementation, and ongoing review of individualized treatment plans.
  • Linking consumers with needed community resources, including housing, medical care, benefits, transportation, employment, education, and other supportive services.
  • Assisting consumers in developing independent living and community integration skills.
  • Providing advocacy, support, education, and crisis intervention as appropriate.
  • Monitoring consumer progress and identifying changes in needs or functioning.
  • Coordinating services with physicians, hospitals, community agencies, family members, and other providers as appropriate.
  • Participating actively in ACT team meetings, treatment planning, and coordination of services.
  • Maintaining accurate, complete, and timely documentation in accordance with FMRS, Medicaid, licensing, and program requirements.
  • Providing transportation and/or accompanying consumers to appointments and community resources when necessary.
  • Working collaboratively with psychiatrists, nurses, therapists, peer recovery support specialists, substance use professionals, and other ACT team members.
  • Participating in crisis response and other ACT coverage responsibilities as assigned.
  • Performing other duties necessary to support the effective operation of the ACT program.

Qualifications

Minimum qualifications:

  • Bachelor's degree in a behavioral health, social services, human services, or related field, or other education/experience combination meeting applicable program and payer requirements.
  • Ability to work effectively with adults experiencing serious mental illness and, frequently, co-occurring substance use disorders.
  • Strong organizational, communication, documentation, and problem-solving skills.
  • Ability to work both independently in community settings and collaboratively as part of a multidisciplinary team.
  • Valid driver's license and ability to meet FMRS requirements for operating a vehicle in the performance of job duties.
  • Ability to successfully complete required background checks, screenings, and agency training.

Experience in community behavioral health, case management, serious mental illness, crisis services, substance use treatment, housing/resource coordination, or ACT services is required.

About ACT

Assertive Community Treatment is a team-based approach designed to provide comprehensive behavioral health treatment, rehabilitation, and support to individuals with significant mental health needs. Rather than expecting consumers to receive all services in a traditional office setting, ACT staff provide many services where the consumer lives and functions in the community.

The ACT Case Manager plays an important role in helping consumers overcome barriers, access needed resources, reduce unnecessary hospitalization, maintain community stability, and work toward personally meaningful recovery goals.

Physical Requirements

1. Frequent communications, verbal and written.

  1. Frequent walking (including stairs) standing and/or sitting.

  2. Visually or otherwise identify, observe and assess consumers.

  3. Ability to perform verbal and nonviolent physical crisis intervention techniques & CPR.

Positions are full time with the typical work week being Monday through Friday, 8am – 5pm and participate in a rotating on-call system. FMRS offers a competitive salary with a broad range of excellent fringe benefits. FMRS offers health, dental and vision insurance, 401 (k) with company match, vacation and sick leave, paid holidays, private disability insurance, and life insurance.
