We're looking for compassionate, enthusiastic, and proactive individuals to join our Active Living Team.

This is a varied and rewarding role where no two days are the same. You'll have the opportunity to support people in their own homes, within our Day Centre, and at our community groups, helping individuals stay active, engaged, and connected.

About the Role

As an Active Living Support Worker, you'll provide person-centred support that promotes independence, wellbeing, and social inclusion.

Your responsibilities may include:

• Providing respite support for unpaid carers
• Supporting individuals attending our Day Centre and community groups
• Planning and delivering personalised activities based on individual interests and care plans
• Assisting with personal care and toileting when required
• Encouraging participation in meaningful activities and social opportunities
• Supporting individuals to access community activities, appointments, and shopping trips
• Assisting with mobility, meal preparation, eating and drinking
• Supporting individuals with medication in line with care plans
• Helping people achieve personal goals and maintain their independence

A Diverse and Flexible Role

This position offers the opportunity to work across several areas of our service, including:

• Supporting individuals in their own homes
• Working within our Day Centre
• Assisting with community groups and activities
• Supporting outings and local trips

There may also be occasions where you will drive our minibus to transport service users safely to activities and outings. Full support and training will be provided. Applicants with a D1 category on their driving licence are particularly welcomed.

Essential Requirements

• Full UK driving licence
• Access to your own vehicle
• Availability 5 days per week between 9:00am and 6:00pm
• Right to work in the United Kingdom

A D1 licence entitlement is desirable but not essential.
