Responsibilities:

• Develop and maintain strong relationships with hospital staff, including case managers, social workers, and physicians, to facilitate efficient and effective discharge planning and coordination
• Communicate with hospital staff to gather information about patients' medical conditions, treatment plans, and discharge needs
• Collaborate with our interdisciplinary care team to ensure that patients' needs are met and that our facility is well-equipped to support their recovery
• Identify potential barriers to discharge and work with the care team to develop strategies to overcome them
• Provide education and support to patients and their families on the discharge process and post-acute care options
• Maintain accurate and timely records of communication with hospital staff and patient information
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

Requirements:

• High school diploma or equivalent required; degree in healthcare-related field preferred
• 4 or more years of experience in healthcare, customer service, or a related field
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work in a fast-paced environment and prioritize multiple tasks
• Strong attention to detail and organizational skills
