Admitting RN

Allstar Home Care is a Medicare-certified home health agency headquartered in Jacksonville, serving patients across Duval, Clay, St. Johns, Flagler, Nassau, Volusia, and Baker counties.

Position Summary
We’re looking for a skilled, efficient home health RN dedicated to patient admissions. As our Admissions RN, you’ll be the first clinician new patients meet: completing thorough Start of Care visits, building accurate assessments, and setting up a plan of care that gets every patient off to a safe, well-coordinated start. Timely, high-quality admissions drive patient outcomes, referral source satisfaction, and accurate reimbursement, which makes this one of the most important roles on our team.

Responsibilities

• Complete Start of Care (SOC) and Resumption of Care (ROC) visits, including comprehensive OASIS assessments
• Admit patients promptly after referral, typically within 24–48 hours of discharge or as ordered
• Review referral packets and discharge records; verify orders, homebound status, and face-to-face documentation, and clarify with physicians as needed
• Perform medication reconciliation and identify high-risk medications and interactions
• Complete admission consents and patient rights education with patients and families
• Assess home safety, fall risk, and risk of hospitalization
• Develop the initial plan of care and coordinate needed disciplines (PT, OT, ST, HHA, MSW)
• Provide skilled interventions at admission as ordered
• Hand off each new admission clearly to the case managing clinician
• Complete accurate, timely documentation in our EMR (Alora Plus) and clock visits through EVV
• Collaborate with intake, liaisons, and clinical leadership to keep admissions moving and referral sources satisfied

Qualifications

• Current, active Florida RN license (or multistate compact license)
• Current CPR/BLS certification
• Minimum 2 years of home health experience, including independent SOC/OASIS completion
• Strong OASIS accuracy and understanding of how assessment drives PDGM reimbursement and quality scores
• IV/infusion experience a plus
• Valid driver’s license, reliable transportation, and current auto insurance;
• Highly organized, efficient, and comfortable with multiple admissions per day
• Comfortable documenting on a smartphone/tablet

Pay: $24.15 - $29.09 per hour
