Advanced Clinical Practitioner

Are you an experienced Advanced Clinical Practitioner looking for an opportunity to make a real difference within urgent and crisis mental health care?

We are looking for a dynamic, experienced and highly skilled ACP to join our Merton Home Treatment Team (HTT) in a permanent 0.5 WTE 18.75hrs Band 8a position, based at Wilson Hospital .

This is an exciting opportunity to work as a senior autonomous clinician within an experienced multidisciplinary Home Treatment Team, supporting people experiencing acute mental health crisis to receive high-quality treatment and care within their own homes wherever possible.

You will be a registered healthcare professional with an MSc or PGDip in Advanced Clinical Practice , an independent prescribing qualification , current professional registration and experience working at an advanced level of practice. Experience within working-age adult community mental health services is an essential requirement of the role.

You will bring strong clinical assessment and risk-management skills, confidence in managing complexity and the ability to work autonomously while building effective relationships across the multidisciplinary team.

The successful candidate will be predominantly based within Merton HTT , with one day per fortnight working within the Crisis Access Hub . This offers a unique opportunity to work across different parts of the crisis pathway, supporting both intensive home treatment and the front end of crisis assessment and clinical decision-making.

The Trust's ACP role encompasses advanced assessment, formulation, diagnosis, treatment planning and non-medical prescribing, alongside clinical leadership, education, research and quality improvement.
