What they need
Job Summary
We are looking for an enthusiastic and experienced Advanced Nurse Practitioner or Advanced Paramedic Practitioner to join our friendly, forward-thinking and innovative team, delivering high-quality, patient-centred care.
We are a well-established Primary Care Network with a strong patient-centred ethos, committed to meeting the evolving needs of our local population while maintaining the traditional values of general practice.
We welcome applications from experienced advanced practitioners who can support us in proactively managing frail and older patients, helping them remain well at home and within the community, while reducing avoidable hospital admissions.
The successful candidate will also be responsible for independently assessing and managing patients presenting with minor illnesses and long-term conditions, providing an effective alternative to GP consultations where appropriate.
Main Duties of the Job
You will be a qualified Advanced Nurse Practitioner or Advanced Paramedic Practitioner, registered with the NMC or HCPC as appropriate, and able to demonstrate the clinical knowledge, skills and experience required to practise autonomously while maintaining excellent standards of patient care.
You will be an independent prescriber and work collaboratively with the wider practice team, community healthcare services, acute healthcare providers and social care professionals to effectively meet the needs of our patients.
Your main duties will include:
- Assessing and managing patients with long-term conditions, including those with complex healthcare needs.
- Independently assessing, diagnosing and managing patients presenting with minor illnesses, within your scope of practice.
- Proactively case managing frail and vulnerable patients, supporting them to remain well and independent within their homes and communities.
- Undertaking comprehensive frailty assessments, clinical reviews and developing appropriate care and management plans.
- Conducting home visits where clinically appropriate, particularly for housebound and vulnerable patients.
- Working collaboratively with the wider multidisciplinary team to coordinate care, support early intervention and reduce avoidable hospital admissions.
- Liaising with community services, hospitals and social care teams to ensure continuity of care and appropriate patient follow-up.
Overview of Our Organisation
Harrow East Primary Care Network (PCN) is a collaborative group of three GP practices committed to delivering high-quality, accessible and patient-centred healthcare to our local community.
We are a forward-thinking and innovative PCN, working closely with our multidisciplinary teams and wider healthcare partners to improve patient outcomes and support the delivery of effective primary care services.
Our organisation offers:
- A supportive and collaborative working environment with a strong patient-centred ethos.
- Established clinical teams including GPs, Advanced Practitioners, Clinical Pharmacists, Practice Nurses, Healthcare Assistants and wider primary care professionals.
- Access to clinical systems including EMIS Web, Docman and AccuRx.
- A strong focus on multidisciplinary team working, particularly in frailty, long-term condition management and mental health.
- Close working relationships with community healthcare services, hospitals and social care providers.
- A commitment to developing efficient clinical workflows and reducing unnecessary administrative pressures on clinicians.
- Opportunities for professional development, collaborative learning and service improvement.
- An established and evolving frailty service supporting patients with complex health and social care needs.
- A competitive salary dependent on qualifications and relevant experience.
We are committed to supporting our clinical teams in delivering effective, coordinated and compassionate care, while continuously improving services to meet the changing needs of our patient population.
