About the Role:

We are looking for an experienced and motivated **Advanced Nurse Practitioner (ANP)** to join our Primary Care Network (PCN) and work collaboratively across our four GP practices.

The successful candidate will work across **one practice per day**, providing a varied clinical service that includes proactive care, long-term condition management, end-of-life care, minor illness and prescribing.

The role will suit an ANP who enjoys working independently, is confident managing a varied clinical caseload and is committed to improving outcomes for patients across the PCN.

Key Responsibilities

* Proactive Care & Complex Patients

* Undertake proactive care reviews and visits for patients identified as requiring additional support.

* Support the management of patients with complex and multiple long-term conditions.

* Carry out holistic assessments and develop appropriate care plans in partnership with patients, families and the wider multidisciplinary team.

* Identify deterioration and escalate appropriately to other members of the clinical team.

* Work proactively to reduce avoidable hospital admissions and improve patient outcomes.

Long-Term Conditions

* Assess, diagnose and manage patients with a range of long-term conditions (LTCs).

* Review medication and treatment plans and optimise management where appropriate.

* Provide patient education, self-management advice and appropriate onward referral.

* Work closely with GPs, practice nurses, pharmacists and other PCN professionals to provide coordinated care.

End-of-Life & Palliative Care

* Provide high-quality end-of-life and palliative care in both practice and community settings.

* Undertake proactive reviews of patients approaching the end of life.

* Support patients and their families with sensitive and compassionate care planning.

* Complete and review Respect (Recommended Summary Plan for Emergency Care and Treatment) forms where appropriate.

* Liaise with community services, palliative care teams, district nurses, hospices and other relevant professionals.

Minor Illness & Same-Day Care

* Assess, diagnose and manage patients presenting with minor illness and acute presentations.

* Provide appropriate treatment, advice and safety-netting.

* Recognise patients requiring escalation and refer appropriately.

* Work within agreed PCN and practice clinical pathways.

Prescribing

* Work as an independent prescriber, undertaking safe and evidence-based prescribing within scope of practice.

* Review and optimise medicines where clinically appropriate.

* Consider polypharmacy, medication interactions and patient-specific risks.

* Work collaboratively with the PCN clinical pharmacy team and wider MDT.

Working Across the PCN

The successful candidate will work across four GP practices within the PCN, generally spending one day at each practice as part of a planned weekly rota.

You will be required to adapt to different practice environments, systems and teams while maintaining a consistent and high standard of clinical care.

A full UK driving licence and access to your own vehicle are essential, as the role will involve travelling between practices and may include proactive patient visits.

* Experienced in long-term condition management.

* Comfortable providing end-of-life and palliative care.

* Experienced or willing to undertake Respect form discussions and care planning.

* Able to work autonomously while recognising when escalation or MDT input is required.

* An excellent communicator who can build trusting relationships with patients, families and colleagues.

* Flexible and able to work effectively across different GP practices.

* Organised, proactive and able to manage a varied workload.

* Committed to delivering safe, compassionate and evidence-based care.

* Supportive multidisciplinary working.

* A role with autonomy and variety across four GP practices.
