Advanced Practice Nurse (PRN)

Now Hiring - Advanced Practice Nurse - Southwest NJ Team PRN

The Advanced Practice Nurse will be responsible for, but are not limited to:

• Conduct re-certification visits to patients on hospice in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with the referring physician, the acute care team, and other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education: Master's Degree in Nursing

Professional Experience:

• Must have a Nurse Practitioner's license
• Must have 2-4 years experience working within hospice or related field
• Must have a Master's Degree
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel
