Los Angeles County Nephrology Associates, part of the Panoramic Health network, is seeking a compassionate and motivated bilingual (Spanish-speaking) Nurse Practitioner or Physician Assistant to join our growing nephrology team in Los Angeles County.

This is an excellent opportunity for experienced providers who interested in building a long-term career in specialty medicine. Our team provides mentorship, collaborative physician support, and hands-on training in nephrology and chronic kidney disease management.

Practice Details

• Majority of this role is focused on clinic with small portion of dialysis or hospital rounding
• Primarily Los Angeles County locations
• Full-time position
• Team-based care model
• Training and mentorship provided

Qualifications

• Licensed Nurse Practitioner or Physician Assistant in California
• Bilingual in Spanish and English required.
• Excellent communication and patient education skills
• Interest in chronic disease management and specialty care
• Ability to work collaboratively in a fast-paced clinical environment

About The Role This provider will work closely with nephrologists and interdisciplinary care teams to evaluate, diagnose, and manage patients with chronic kidney disease, hypertension, dialysis needs, and related medical conditions.

Responsibilities Include

• Conducting patient assessments and physical examinations
• Developing treatment plans
• Managing medications and chronic conditions
• Documenting care accurately within the EMR
• Educating patients and families on treatment plans and disease management
