  • Review laboratory results, medications, treatment response, and changes in clinical condition.
  • Provide patient and family education regarding kidney disease, dialysis, medications, and treatment plans.
  • Coordinate care with physicians, dialysis facilities, office staff, and other members of the patient's healthcare team.
  • Maintain timely and accurate documentation in the electronic medical record.
  • Communicate effectively through established clinical and electronic communication systems.
  • Participate in ongoing coordination of care to support continuity and timely follow-up for nephrology patients.

  • Must be comfortable managing patients with chronic and complex medical conditions in both outpatient and dialysis settings.
  • Strong clinical assessment, critical thinking, and care coordination skills are essential.
  • Ability to work collaboratively with the supervising/collaborating physician while also functioning independently within the APP's scope of practice.
  • Must demonstrate strong organizational and communication skills and the ability to reliably manage a schedule across multiple clinical settings.
  • Proficiency with electronic medical records and timely clinical documentation is required.
  • Candidates without nephrology experience should have a strong interest in kidney disease and willingness to develop specialized nephrology knowledge.

Certification & Licensure

  • Candidates must meet applicable Hawaii licensure requirements for their discipline and maintain:
  • Current Hawaii licensure as a Nurse Practitioner/APRN or Physician Assistant, as applicable.
  • Current national certification through ANCC or AANP for Nurse Practitioners or NCCPA for Physician Assistants.
