About Signe Spine & Rehab

Signe Spine & Rehab (SSR) is a physician-led physical medicine and rehabilitation practice serving the Charleston area. Led by board-certified physiatrist Daniel Nemeth, MD, our team focuses on finding the underlying causes of pain, restoring function, and helping patients return to meaningful activity through individualized, comprehensive care.

Position Summary

We are seeking a full-time, motivated, clinically thoughtful Physician Assistant or Nurse Practitioner to join a growing practice in a blended outpatient and inpatient rehabilitation role. Approximately 65% of the position supports outpatient interventional spine and pain management at SSR; Approximately 35% of the role involves supporting Dr. Nemeth in the care and management of patients admitted to a local hospital’s acute inpatient rehabilitation unit. These patients typically have relatively low medical acuity and require short-term inpatient rehabilitation stays. This role combines hands-on patient care, interdisciplinary coordination, accurate documentation, and opportunities to help improve workflows and grow services.

We value someone who takes ownership of patient care, follows through without constant prompting, recognizes opportunities to improve the patient experience, and is energized by helping a small, physician-led practice grow. The right candidate brings initiative and accountability while respecting teamwork, scope of practice, and sustainable work expectations.

Key Responsibilities | Outpatient Clinic (approximately 65%)

  • Assess, diagnose, and manage patients with acute and chronic musculoskeletal, spine-related, and pain conditions through histories, physical examinations, imaging/diagnostic review, and individualized treatment planning.
  • Monitor outcomes and adjust conservative and interventional care plans in collaboration with Dr. Nemeth; counsel patients on treatment options, function, recovery, and self-management.
  • Diagnostic Imaging Review and Interpretation: Review and interpret in-house X-rays and externally obtained MRI studies, including evaluating imaging findings beyond the radiologist’s report. Correlate findings with the patient’s clinical presentation, review and explain images directly with patients, and incorporate findings into clinical assessments and treatment planning in collaboration with the supervising physician. Prior experience with advanced imaging interpretation is not required; training and physician guidance will be provided as needed to develop proficiency in these responsibilities.
  • Review and manage medications, reconcile medication lists, and monitor safety and response to treatment within authorized prescribing scope.
  • Perform appropriate musculoskeletal injections (e.g., knee, shoulder, subacromial bursa, greater trochanteric bursa, trigger point) based on competency, privileging, and practice needs.
  • Complete timely, thorough clinical documentation; support accurate coding, charge capture, and medically necessary services.
  • Manage and respond to patient inquiries and clinical messages, review physical therapy plans of care and incoming imaging reports, complete insurance authorization peer-to-peer reviews, and assist with disability documentation and other clinical administrative tasks to ensure timely, coordinated patient care.
  • Discuss complex cases with Dr. Nemeth, participate in patient review conferences, and coordinate care with referring clinicians, physical therapists, and other providers.
  • Provide clear patient education, address concerns respectfully, communicate with clinical support staff, and support safe, efficient clinic operations.

Key Responsibilities | Inpatient Rehabilitation (approximately 35%)

  • Support Dr. Nemeth with daily clinical management of patients admitted to an acute inpatient rehabilitation unit, adjusting to patient census, acuity, and coverage needs.
  • Perform appropriate follow-up evaluations, review clinical status and progress, and prepare timely progress notes and other required medical documentation.
  • Reconcile, review, and manage medications; monitor response and adverse effects; coordinate medical recommendations with internal medicine hospitalists and the attending physician.
  • Assess and address common rehabilitation issues, including bowel and bladder function, pain control, sleep, mobility-related symptoms, and other recovery needs.
  • Coordinate nutritional concerns and recommendations with hospitalists, nursing, dietitians, and other care team members as appropriate.
  • Enter or prepare clinically appropriate medication, therapy, discharge, and related orders and instructions, consistent with privileges, hospital policy, and physician requirements.
  • Collaborate with physical and occupational therapy, nursing, case management, and the broader rehabilitation team regarding therapy plans, barriers to discharge, and functional goals.
  • Support discharge readiness, medication reconciliation, discharge instructions, follow-up arrangements, and timely communication with patients and families.
  • Escalate changes in condition, complications, or higher-acuity issues promptly to Dr. Nemeth and the appropriate hospital team.

Initiative, Collaboration & Practice Growth

  • Take ownership of assigned patients and tasks from assessment through follow-up, proactively identifying gaps and closing the loop.
  • Help improve scheduling, documentation, patient education, clinical protocols, care transitions, and collaboration between clinic and hospital teams.
  • Contribute ideas to improve patient access, physician efficiency, quality outcomes, and sustainable growth of the practice.
  • Build strong relationships with referring physicians, therapists, hospital partners, and the community while maintaining professional and ethical standards.
  • Participate in quality assurance, utilization review, infection prevention, safety efforts, and other practice improvement initiatives.

Required Qualifications

  • Graduate of an accredited PA or NP educational program; current, unrestricted South Carolina PA or APRN license and applicable national certification.
  • Current BLS certification; DEA and applicable South Carolina controlled-substance registrations when required for assigned duties.
  • Collaborate with the Practice Manager to obtain and maintain all required hospital credentials and privileges, collaborative/scope-of-practice arrangements, and payer enrollment, ensuring timely completion of necessary documentation and ongoing compliance.
  • Strong clinical judgment, clear documentation, excellent communication, and ability to coordinate care across outpatient and hospital teams.

Preferred Qualifications

  • Experience in PM&R, inpatient rehabilitation, hospital medicine, orthopedics, neurosurgery/spine, or interventional pain management.
  • Comfort with post-operative rehabilitation, medication reconciliation, bowel/bladder management, interdisciplinary rounds, discharge planning, and electronic health records.
  • Strong proficiency in musculoskeletal and spinal imaging, including independent review of cervical, thoracic, and lumbar MRI studies; ability to identify relevant degenerative, discogenic, stenotic, and other spinal pathology and correlate imaging findings with clinical symptoms.
  • Proactive, adaptable approach to problem-solving and interest in participating in practice development.

Schedule & Employment Details

This is a full-time position, generally expected to average 40 hours per week. Approximately 65% of time (about 26 hours per week) will support outpatient care at Signe Spine & Rehab and 35% (about 14 hours per week) will support Dr. Nemeth at the local inpatient rehabilitation unit. Actual hours and daily distribution may vary with hospital census, patient care requirements, clinic schedules, and operational needs. The detailed work schedule, any weekend or on-call expectations, compensation, and benefits will be confirmed during the hiring process.

Professional Expectations & Compliance

All care, orders, prescribing, procedures, documentation, and billing must comply with South Carolina law, applicable physician collaboration or supervision requirements, CMS rules, hospital bylaws and granted privileges, SSR policies, HIPAA, and professional standards. The APP works collaboratively with Dr. Nemeth and the hospital team.

This position description outlines the general nature of the position and may be updated as practice and hospital needs evolve. Signe Spine & Rehab is committed to fair employment practices.
