About the Role
We are seeking a qualified Advanced Practice Provider (APP) to provide patient-centered care through telehealth video visits and asynchronous follow-up for hormone health, HRT/TRT, medical weight management, GLP-1 therapy, and peptide-related services. Candidates must be qualified as either a Physician Assistant (PA) or Nurse Practitioner (NP). Care may be provided across multiple states only where the APP is appropriately licensed, credentialed, and legally authorized to practice.
The APP will assess patients, review advanced laboratory panels and biomarkers, develop individualized treatment plans, and manage ongoing clinical follow-up. Responsibilities include medication management, laboratory monitoring, and patient follow-up as needed.
The APP will work directly with the Medical Director to apply and refine clinical protocols as the practice grows. The sales and operations teams will support patient coordination and administrative needs so the APP can focus primarily on clinical responsibilities. The ideal candidate is committed to patient safety, evidence-informed care, clear communication, and compliance with applicable professional and regulatory standards.
Key Responsibilities
- Conduct telehealth video consultations, clinical assessments, and follow-up care for patients located in states where the APP is appropriately licensed and authorized to practice; provide asynchronous follow-up when permitted and clinically appropriate.
- Review medical histories, symptoms, medications, advanced laboratory panels, and relevant biomarkers to inform clinical decision-making.
- Develop individualized treatment plans for HRT/TRT, medical weight management, and other applicable services, including GLP-1 medications and peptide-related therapies.
- Educate patients on treatment options, expected outcomes, potential risks, side effects, alternatives, and ongoing care requirements.
- Prescribe and manage medications within the APP’s authorized scope, applicable state requirements, and required supervision, collaboration, or practice arrangements.
- Maintain accurate, timely, and confidential patient records and address laboratory results, patient messages, and clinical concerns within established timeframes.
- Work directly with the Medical Director and collaborate with other clinicians to apply, evaluate, and help refine clinical protocols and standards of care.
- Coordinate with sales and operations personnel to support appropriate patient handoffs and a smooth patient experience while maintaining independent clinical judgment and professional boundaries.
- Comply with applicable federal and state laws, licensure and telehealth requirements, prescribing standards, privacy obligations, and professional practice guidelines.
KPIs / Performance Expectations
- Complete video consultations, follow-up care, and asynchronous patient communications in a timely and professional manner.
- Maintain accurate, complete, and timely clinical documentation for all patient interactions.
- Deliver clear, patient-centered education regarding treatment plans, medications, risks, alternatives, and ongoing care.
- Base clinical recommendations on patient history, symptoms, laboratory findings, applicable protocols, and independent clinical judgment.
- Review and address laboratory results, patient messages, and clinical concerns within established timeframes.
- Maintain compliance with applicable clinical, privacy, telehealth, licensure, supervision or collaboration, and prescribing requirements.
- Communicate effectively with the Medical Director, clinicians, sales team, and operations personnel to support coordination and continuity of care.
- Contribute to the development and maintenance of accurate clinical protocols, standards, workflows, and patient education materials.
- Identify opportunities to improve clinical workflows, patient communication, safety, and the overall quality of care.
Experience
- At least two years of hands-on clinical experience in HRT/TRT, medical weight management, and/or peptide-related therapy.
- Experience conducting patient consultations, interpreting advanced laboratory panels and biomarkers, and developing individualized treatment plans.
- Experience prescribing and managing hormone therapies, GLP-1 medications, or other weight-management therapies within an authorized scope of practice.
- Familiarity with electronic health records, telehealth platforms, video visits, and asynchronous virtual-care workflows.
- Ability to collaborate with a Medical Director, supervising or collaborating physicians where required, other clinicians, and operational teams while maintaining appropriate clinical judgment.
- Preferred: Functional or integrative medicine training, such as IFM, A4M, or similar education, and experience developing clinical protocols or quality-improvement initiatives.
- Preferred: High-volume telehealth experience; active PA or NP licenses in multiple states, with 30 or more active state licenses considered a strong advantage; and experience with compounded medications or peptide protocols within applicable legal and clinical requirements.
Qualifications
- Graduate of an accredited Physician Assistant or Nurse Practitioner program, with the applicable professional certification required for the role.
- For PA candidates: Current NCCPA certification and an active, unrestricted PA license in every state where care will be provided.
- For NP candidates: Current national NP certification through an applicable recognized certifying body and an active, unrestricted NP license in every state where care will be provided.
- Compliance with applicable state practice, telehealth, patient-location, supervision, collaboration, and prescribing requirements for the candidate’s profession and each jurisdiction served.
- Active DEA registration and any state controlled-substance registrations, prescriptive authority, or other credentials required for the therapies and jurisdictions within the agreed scope of services.
- Current BLS certification and any additional credentials required for the position or applicable jurisdictions.
- Strong clinical judgment, communication skills, attention to detail, and commitment to patient-centered care.
- Demonstrated knowledge of HIPAA requirements, informed consent, clinical documentation, professional boundaries, and secure handling of protected health information in telehealth and clinical settings.
- Commitment to evidence-informed care, patient safety, ethical prescribing, and use of only legally permitted and clinically appropriate therapies.
- Strong organization and time-management skills, with the ability to manage a patient panel in a remote environment.
Work Arrangement & Compensation
- Independent Contractor (1099) part-time position, approximately 15–20 hours per week, with responsibilities and scheduling outlined in a written agreement.
- Remote/telehealth delivery of care, limited to patients located in states where the APP is appropriately licensed, credentialed, and authorized to practice and prescribe.
- Patient volume, scheduling, availability, and asynchronous coverage expectations to be established based on business needs and the agreed-upon scope of services.
- Compensation to be determined based on experience, credentials, licensure footprint, and scope of responsibilities.
How to Apply
Interested candidates should submit a current resume or CV outlining their professional education, applicable PA or NP certification, active licenses, DEA registration, and relevant clinical experience. Applicants are encouraged to highlight multi-state licensure; telehealth and asynchronous-care experience; HRT/TRT, medical weight management, GLP-1, and peptide-related experience; advanced laboratory interpretation; functional or integrative medicine training; and ongoing patient panel management.
