AFC Program Director

Job Summary

Compass AFC is seeking an experienced Program Director to lead our Adult Foster Care program. If you have worked in a Massachusetts LTSS program and know what it takes to run a compliant, client-centered operation, we want to hear from you.

In this role, you will oversee day-to-day program operations, lead and develop our team, and keep the agency aligned with MassHealth and accreditation standards. You will also guide the program's growth, building strong relationships with referral sources, health plans, and community partners. Your work will help older adults and adults with disabilities stay safely at home with the caregivers they trust.

Responsibilities

• Plan, direct, and oversee all AFC program operations
• Develop and implement policies and procedures that meet MassHealth AFC regulations and CARF standards
• Keep the program audit-ready, with accurate documentation and timely reporting
• Lead hiring, training, supervision, and performance evaluation for all staff
• Build systems for recruitment, retention, and ongoing staff development
• Develop and manage the annual budget, monitor expenses, and prepare financial reports
• Ensure caregiver stipend payments are accurate and on time
• Lead outreach and build relationships with referral sources, health plans, and community partners
• Manage contracts and agreements with payers and vendors
• Use data to track program outcomes and drive quality improvement
• Report to the Board of Directors on program performance and growth opportunities

Qualifications

Required

• Bachelor's degree and 5+ years of healthcare experience serving older adults or people with disabilities, or a Master's degree in a healthcare-related field and 3+ years of that experience
• At least 3 years in an administrative or management role
• Knowledge of state and federal regulations for home and community-based services
• Strong leadership, communication, and writing skills
• Proficiency in Microsoft Office (Word, Excel, Outlook), Google Drive, and AI.

Preferred

• Experience in a Massachusetts AFC, GAFC, or Adult Day Health program
• Familiarity with MassHealth regulations, authorizations, and audits
• Experience with CARF accreditation
• Budgeting and business management experience
