Aged Care Coordinator

We are seeking an AN-ACC Coordinator to support multiple residential aged care homes, ensuring assessments are accurate, funding opportunities are maximised, and compliance standards are consistently achieved.

The Role

• Based in Rivervale
• Lead AN-ACC processes across multiple residential aged care homes
• Review and support assessment accuracy and documentation quality
• Identify opportunities to optimise AN-ACC funding outcomes
• Support compliance with aged care funding requirements
• Partner with site leadership teams and key stakeholders
• Undertake regional travel as required

About You

• 3-5 years' experience with AN-ACC funding processes
• Strong understanding of the AN-ACC funding model
• Experience within residential aged care
• Excellent communication and stakeholder engagement skills
• Strong computer literacy and attention to detail
• Current driver's licence and willingness to travel
