About the Role

As a Community Home Care Support Worker, you will provide personalised support to clients living in their own homes, helping them maintain their independence and quality of life.

Your Responsibilities

• Assist with personal care, including showering, dressing, grooming, and mobility support
• Support daily living activities such as meal preparation, medication reminders, shopping, and light household tasks
• Provide companionship and social support
• Assist clients to attend appointments and participate in community activities
• Promote independence, choice, and dignity in all aspects of care
• Maintain accurate documentation and progress notes
• Build positive relationships with clients, families, and healthcare professionals

About You

You are caring, dependable, and passionate about supporting others.

You'll bring:

• Certificate III or IV in Individual Support, Aged Care, Home and Community Care, Disability Support, or a related qualification (or currently studying)
• Previous experience in community care, home care, aged care, disability support, or a similar role
• Excellent communication and interpersonal skills
• A compassionate and client-focused approach
• Strong time management and organisational skills
• Ability to work independently and as part of a team

Essential Requirements

• Current National Police Check
• Current First Aid and CPR Certificate
• Current Working with Children Check (or willingness to obtain)
• Valid Australian Driver's Licence
• Reliable and comprehensively insured vehicle
• NDIS Worker Screening Check (desirable)
