Annual Wellness Coordinator

Duties/Responsibilities:

· Identify and maintain lists of patients who may be eligible for Annual Wellness Visits and other preventive services.

· Contact eligible patients to explain and schedule AWVs.

· Coordinate AWV appointments with provider schedules and clinic availability.

· Monitor scheduled, completed, rescheduled, and missed AWVs.

· Follow up with patients who need to reschedule or complete outstanding preventive services.

· Help ensure appropriate documentation and information are available prior to the patient's visit.

· Conduct telephone and other approved outreach to eligible patients.

· Explain the purpose and general process of an Annual Wellness Visit.

· Prepare appropriate forms, questionnaires, and documentation for scheduled visits.

· Assist with identifying gaps in preventive care based on available records and established processes.

· Conduct nursing components of Annual Wellness Visits in accordance with established protocols and provider direction.

· Complete and document appropriate health risk assessments.

· Obtain and document patient history, medications, allergies, vital signs, and other relevant information.

· Review available preventive care and health maintenance information.

· Perform nursing assessments within the RN's scope of practice.

· Other duties as assigned.

Qualifications:

· High school diploma or equivalent required.

· Previous healthcare, medical office, care coordination or administrative experience preferred.

· Excellent verbal and written communication skills.

· Strong attention to detail and follow-through.

· Previous experience with Annual Wellness Visits or preventive care programs preferred.

· LPN or RN license required.

Pay: $52,000.00 - $62,400.00 per year
