What they need
JOB PURPOSE:
The Application Developer III supports the building of the organization's multi-tenant healthcare SaaS platform on Microsoft Azure. The platform serves multiple enterprise clients and supports daily operations, including member enrollment and eligibility checks, scheduling, care-coordination workflows, document management, configurable forms and assessments, letters, reporting, and team communication. The role helps ensure each tenant's data remains strictly isolated and supports secure data exchange with payers and other vendors through APIs and integrations.
The position contributes across the whole stack, including APIs, databases, front-end, cloud infrastructure, and AI features, while setting technical standards for the development team. This senior full-stack development role supports the advancement of a rapidly growing healthcare platform that is in active development.
JOB RESPONSIBILITIES:
Build features end to end. The back end is an ASP.NET Core API that uses CQRS with MediatR, FluentValidation, and EF Core. The front end is TypeScript, built with React Native for Web (Expo), Redux Toolkit, and RTK Query.
Drive the implementation and evolution of our multi-tenant architecture, with strict data isolation between tenants, strong access controls, and efficient use of resources.
Build secure, versioned REST APIs with:Microsoft Entra ID / JWT authentication
role- and capability-based access control
field-level permissions
checks that a user may access each specific record
audit logging that meets HIPAA requirements
Build and maintain integrations: X12 EDI 270/271 eligibility, e-signature, contact-center phone systems, SFTP file exchange, and Microsoft Graph. These run on Azure Service Bus, Azure Functions, and background services.
Extend the platform's workflow automation engine, which runs on events and schedules and is built on Durable Task. Work on real-time features (SignalR) for notifications, chat, and live updates.
Build AI features with Azure OpenAI, keeping PHI exposure to a minimum and a person in the loop:an assistant that can call tools
document search that answers with citations (Azure AI Search)
AI sorting of incoming documents
live speech transcription (Azure Speech) that suggests form answers for a user to accept
Design SQL Server schemas with our data team, write and tune complex queries, and fix EF Core performance problems on large tables.
Ship through GitHub Actions to Azure: container images on App Service (via Azure Container Registry), Azure Functions and Azure Static Web Apps, monitored with Application Insights.
Review code, mentor junior and mid-level developers, and raise the bar on testing (xUnit, Moq, Jest, React Testing Library), documentation and architecture.
Work closely with product owners, the data team, and other engineers in an Agile team: sprint planning, stand-ups, backlog refinement, and retrospectives.
Solve hard production problems anywhere in the stack, from slow screens and query plans to authorization failures and Azure configuration.
Schedule: 8:30AM – 5:30PM
Weekly Hours: 40
Location: On-Site, 136-65 37th Avenue, Flushing, NY 11354
QUALIFICATIONS:
Education:
Bachelor’s degree in Computer Science or related field.
Required Skills & Experience
Backend
7+ years building production systems with C# and ASP.NET Core, including large-scale, distributed systems.
Strong REST API design and security: OAuth 2.0 / OpenID Connect, JWTs, API keys, validation, error handling, and versioning. You know how to prevent unauthorized record access (IDOR) and leaking internal details in error messages.
Strong SQL Server skills: schema design, complex queries and stored procedures, indexing, execution plans and tuning for high data volumes.
Production experience with Entity Framework Core, including mapping to existing database-first schemas, query performance, and migrations.
Experience with clean, layered architectures: CQRS/MediatR, repository and unit-of-work patterns, dependency injection.
Frontend
5+ years with React: function components and hooks, state management with Redux Toolkit / RTK Query or similar, and performance tuning for large, data-heavy applications.
Expert TypeScript and modern JavaScript (ES6+), including async patterns.
Responsive, accessible UI built on a shared design system and utility CSS such as Tailwind.
Experience connecting complex APIs to the UI: loading, error, and permission states, real-time updates, and server-side paging, sorting, and filtering.
Azure
Hands-on experience with Azure App Service (including container deployments), Azure SQL, Azure Functions, Service Bus, Blob Storage, and Key Vault.
Microsoft Entra ID sign-in and authorization for web apps and APIs.
Monitoring and troubleshooting with Azure Monitor and Application Insights.
Familiarity with Docker and CI/CD pipelines (GitHub Actions preferred).
AI
Experience shipping LLM features in production apps, ideally on Azure OpenAI: prompt design, tool/function calling, retrieval-augmented generation (RAG), and handling model failures gracefully.
Understanding of how to handle sensitive data (PHI) safely in AI features, in line with HIPAA.
Testing & Tooling
Unit testing with xUnit (or NUnit) and Moq, and front-end testing with Jest and React Testing Library. A practical approach to integration and end-to-end testing.
Swagger/OpenAPI and API tools such as Postman.
Expert Git skills in a branch-and-pull-request workflow.
Front-end tooling such as npm/pnpm and modern bundlers (Metro, Webpack, or Vite).
Preferred Qualifications:
Proven track record of building and deploying complex multi-tenant SaaS applications at scale.
React Native and Expo, especially React Native for Web; NativeWind and Gluestack UI.
Healthcare experience: HIPAA, PHI handling, Medicaid/Medicare eligibility, X12 EDI (270/271), HL7 or FHIR.
Workflow and messaging systems: Durable Task / Durable Functions, Service Bus, outbox and idempotency patterns.
Real-time applications with SignalR.
Azure AI Search, Azure AI Speech, or document-processing and PDF-generation pipelines.
Python for automation, data processing, or AI/ML work.
Experience leading Agile ceremonies or improving team processes.
Open-source contributions or active involvement in technical communities.
Soft Skills
Excellent problem-solving and debugging skills; you find the root cause, not just the symptom.
Clear written and spoken communication with both technical and non-technical people.
A natural mentor who builds a collaborative, supportive, high-performing team.
Self-motivated and always learning, including new technologies and the healthcare domain.
Able to influence technical decisions and drive best practices across the team.
