APRN/PA – Post-Acute and Long-Term Care – Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant providing independent, comprehensive medical care in long-term care facilities.

Responsibilities include:

• Conducting physical exams, diagnosing, treating illnesses, prescribing medications, and managing chronic conditions.
• Providing preventive care and health counseling.
• Collaborating with facility staff, leadership, and external providers to ensure holistic patient care.
• Maintaining compliance with regulations, accurate documentation, and participating in quality initiatives.
• Overseeing infection control, health promotion, and safety protocols.
• Working independently with strong clinical decision-making and communication skills.
• Utilizing EHR and telemedicine tools.

Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and board certification. The role is in-person within statewide facilities, offering benefits such as health insurance, retirement plans, and paid time off.
