You will be responsible for reviewing files, conducting virtual or face-to-face assessments, and writing high-quality, strengths-based reports that clearly capture a participant's life situation and needs. Please note: This role focuses strictly on assessment and reporting; you will not make funding, budget, or support hour determinations.
Key Responsibilities:
- Conduct and Synthesise Assessments: Gather information directly from participants and their representatives, analysing and interpreting external evidence to make evidence-based recommendations.
- Report Writing & Case Management: Prepare clear, accurate, objective, and defensible written assessment reports using digital case management tools.
- Person-Centred Communication: Communicate clearly, respectfully, and empathetically through both face-to-face and virtual channels, utilising trauma-informed and culturally aware approaches.
- Collaboration & Escalation: Work across teams to support planning processes, identifying and escalating complex risks or policy limitations when necessary.
Skills & Experience Required:
- Mandatory Qualifications: Recognised tertiary qualification in an Allied Health discipline (e.g., Occupational Therapy, Speech Pathology, Social Work, Physiotherapy, Counselling, Exercise Physiology) OR a relevant Health qualification (RN, EN, or NDIS Behaviour Support Practitioner).
- Assessment Expertise: Proven experience administering support needs assessments or utilising person-centred/strengths-based assessment frameworks. Experience with semi-structured interviews is highly regarded.
- Communication & Writing: Advanced interpersonal skills to navigate complex, sensitive conversations, paired with strong analytical writing skills to produce clear, defensible reports.
- Clinical Practice: A deep commitment to trauma-informed, culturally safe, and person-centered practices.
- Tech Literacy: Digital literacy to navigate CRM case management systems and conduct video meetings (e.g., Microsoft Teams).
Highly Regarded:
- Accreditation or practical experience using the I-CAN Support Needs Assessment Tool (desirable, not essential).
