Job description:
ABOUT COMPANY
NHG Population Health, part of NHG Health, is committed to improving the health and wellbeing of 1.5 million residents in Central and North Singapore. Our work goes beyond hospitals, into neighbourhoods, schools and workplaces, by bringing care closer to where people live.
Through our three Missions - Population Health, Community Health and Primary Care — we partner hospitals, polyclinics, general practitioners, and community organisations to provide integrated, person-centred care across all life stages.

This includes partnering with Right-Siting Officers (RSOs) and clinical teams to facilitate the timely and appropriate transition of patients from hospitals to primary care and community care providers.
