JOB SUMMARY

The Associate of Adjacent Systems Pathways will be critical to developing partnerships with

external stakeholders, establishing effective workflows, and referral pathways that reduce homelessness. Experience with evaluation and process improvement is important to determining the efficacy of our operating rhythms and recommending change to best support the people we serve.

JOB RESPONSIBILITIES

  • Independently analyze program, referral, utilization, and outcome data to identify trends, performance gaps, inequities, and emerging risks.
  • Validate quantitative findings through partner engagement, workflow analysis, and stakeholder feedback to distinguish symptoms from root causes.
  • Translate findings into prioritized recommendations, corrective-action plans, coaching strategies, and system-level interventions.
  • Coach and redirect partner agencies using performance data, contractual expectations, and system goals; document commitments, monitor follow-through, and escalate unresolved barriers.
  • Prepare concise, decision-ready updates for CFTH leadership that explain the issue, contributing factors, options considered, recommended action, anticipated impact, and implementation risks.
  • Establish performance indicators and evaluate whether implemented solutions produce intended results; recommend whether initiatives should be sustained, modified, expanded, paused, or discontinued.
  • Lead complex, cross-functional workgroups with structured decision-making, clear accountability, and measurable outcomes.
  • Challenge ineffective practices constructively and develop, test, and refine innovative and scalable solutions when established approaches do not meet system goals.
  • Influence stakeholders across organizations without direct supervisory authority while maintaining productive and accountable partnerships.
  • Manage multiple complex initiatives with clear scopes, milestones, dependencies, risks, and follow-through practices.
  • Build, strengthen, and sustain strategic partnerships with healthcare, justice, supportive-service, and other adjacent systems to improve coordinated access and client outcomes.
  • Analyze where clients are delayed, diverted, or lost between systems and determine the policy, process, capacity, data, or partnership factors contributing to breakdowns.
  • Develop and negotiate cross-system referral, discharge, eligibility, handoff, service-expectation, and escalation protocols that clarify ownership and improve implementation consistency.
  • Track pathway utilization, referral completion, timeliness, and client outcomes; use findings to recommend strategic pathway enhancements and resource priorities.
  • Partner with internal teams to design, test, evaluate, and scale the CFTH Rapid Exit referral platform as a coordinated-access tool.
  • Assess supportive-service pathways for accessibility, appropriateness, capacity, utilization, equity, client fit, and gaps; translate findings into actionable improvement strategies.
  • Maintain a current inventory of active pathways and supportive-service resources, including eligibility, capacity, referral requirements, risks, and strategic use cases.
  • Monitor assigned subrecipient contract outputs and data quality, diagnose performance gaps, recommend corrective action and training priorities, and communicate material risks upward.
  • Create provider-facing tools, workflow documentation, and templates that clarify expectations, strengthen navigation, and improve consistent execution across service connections.

QUALIFICATIONS

Educational, Experience, and Skills

  • Bachelor's degree in public administration, social services, data analytics, business, public health, or a related field and at least four (4) years of progressively responsible relevant experience; or an equivalent combination of education and at least six (6) years of relevant experience.
  • Working knowledge of fair housing laws and regulations, HUD guidelines and funding requirements, and HUD Data Standards.
  • Strong written and verbal communication skills, attention to detail, and the ability to prioritize multiple complex responsibilities in a fast-paced environment.
  • Ability to work independently, collaborate cross-functionally, and engage effectively with diverse stakeholders.
  • Demonstrated experience independently analyzing quantitative and qualitative data and translating findings into operational and strategic recommendations.
  • Demonstrated success diagnosing complex problems, identifying root causes, developing multiple solution options, and implementing measurable improvements.
  • Experience coaching, advising, or redirecting external partners based on performance evidence, contractual expectations, and shared system goals.
  • Experience leading cross-functional or cross-organizational initiatives and influencing outcomes without direct supervisory authority.
  • Ability to prepare and present concise, decision-ready analyses to senior leaders, including findings, contributing factors, risks, options, recommendations, and anticipated impact.
  • Demonstrated ability to challenge established practices constructively, navigate ambiguity, and design innovative, feasible, equitable, and scalable approaches.
  • Strong facilitation, consultation, negotiation, stakeholder accountability, and change-management skills, including the ability to conduct difficult performance conversations.
  • Proficiency using Excel, dashboards, HMIS reports, or comparable tools to identify trends, examine performance, communicate findings, and monitor corrective actions.
  • Demonstrated ability to establish outcome measures, assess whether an intervention worked, and adjust strategy based on evidence and stakeholder feedback.
