Associate (Cares)

About the Role

We are looking for a committed and motivated Associate (Cares) to join our team at United Medicare Pte Ltd, a private healthcare provider. This full-time role supports the Executive (Cares), by assisting in case management, collecting resident/family information and providing key social insights to the CARES and clinical teams to facilitate effective care planning.

What you'll be doing

• Collect social and financial information from residents and their families to support applications for financial assistance schemes
• Update and maintain accurate records in the referral, enquiries, and information management systems
• Ensure timely submission of financial assistance applications, including Medifund, MFEC, IDAPE, PGDAS, and other relevant schemes
• Serve as the primary point of contact for residents, families, and healthcare institutions to coordinate referrals and process applications for potential residents
• Maintain proper documentation of case files, referrals, and related records to support case management and audit requirements
• Perform data entry and regularly update databases to ensure accuracy and completeness
• Handle administrative duties, including managing enquiries, referrals, and admissions
• Facilitate transfers to other facilities or healthcare institutions and support discharge processes for residents
• Perform any other duties or projects assigned as required

What we're looking for

• ‘O’ Level or Diploma qualification
• Service-oriented, with good interpersonal and communication skills
• A dependable and collaborative team player
• Proactive, self-motivated, and able to work independently
