Bilingual (Spanish) Stabilization Case Manager

Overview
Join our team as a Bilingual (Spanish) Stabilization Case Manager and make a meaningful impact in the lives of individuals experiencing housing instability. In this role, you will offer support, resources, and advocacy to help clients achieve and maintain their housing and wellbeing. Please note our participants include individuals with mental illness, long histories of substance use, and limited experience with independent living.

This position prioritizes effective written and oral communication with a diverse group of clients, providers, and landlords, experience navigating complex situations in a trauma-informed manner, and attention to detail. Your role is especially essential in assisting clients who are primarily Spanish-speaking access and navigate various systems and resources. This position offers an exciting opportunity to work independently and as part of a team in a caring environment dedicated to social justice, housing equity, and client empowerment.

Responsibilities

• Assist unhoused and formerly unhoused individuals become independent and successful by helping them obtain permanent housing and providing follow-up case management services.
• Help participants obtain and maintain mainstream benefits (e.g. SNAP, SSI, MassHealth, etc.) and facilitate participants' transition from homeless-based to community-based resources.
• Serve as liaison for clients by negotiating with landlords, property management companies, and service providers to secure or maintain housing arrangements.
• Assist participants in relocating as needed.
• Establish and monitor individual service plans, including mental health and substance use reports.
• Support clients in accessing detox or other appropriate substance use treatment.
• Provide mental health or other crisis management support (e.g. de-escalation strategies, harm reduction, creating safety plans, accessing emergency services, motivational interviewing, suicide assessment and intervention, etc.)
• Travel throughout Greater Boston conducting home visits.
• Assist with budgeting skills and other activities of daily living.
• Connect with legal resources and assist clients during eviction processes in housing court.
• Maintain accurate and up to date paper and digital client files (e.g. notes, releases, etc.) and relevant data entry.
• Make referral as needed to relevant client resources or programs.
• Other duties as assigned or as program changes occur.
