What they need
Fort Lauderdale Vet Center is looking for a Care Coordination Manager who can help veterans access the support, resources, and services they need while ensuring communication and follow-through remain consistent.
Veterans often work with multiple organizations, providers, and support systems. Successful coordination requires someone who can understand individual needs, organize next steps, and help remove barriers along the way.
The Care Coordination Manager will help connect veterans with appropriate resources while supporting a smooth, respectful, and responsive service experience.
What you'll be responsible for
- Coordinate support services and resources for veterans and clients
- Help assess needs and identify appropriate next steps or available resources
- Serve as a point of contact between clients, providers, community partners, and internal teams
- Track referrals, requests, and follow-up activities
- Maintain accurate client records and service documentation
- Help ensure clients understand available programs and support options
- Coordinate communication between multiple service providers or organizations
- Monitor ongoing cases and help address barriers to receiving services
- Support outreach, resource connections, and community partnerships
- Identify recurring service challenges and help improve coordination processes
- Maintain confidentiality and handle sensitive information appropriately
- Support a client-centered approach focused on respect, dignity, and responsiveness
What we're looking for
- Experience in care coordination, case management, healthcare services, behavioral health, social services, veteran programs, or a related field
- Strong communication and relationship-building skills
- Compassionate and professional approach when working with veterans and families
- Ability to manage multiple cases, priorities, and deadlines
- Strong organization and attention to detail
- Comfortable coordinating with healthcare providers, community organizations, and support services
- Experience maintaining records and documentation
- Familiarity with healthcare, social-service, or community resources is helpful
- Veteran services experience is preferred, but not required
You don't need to come from one specific organization.
Someone who has coordinated care in healthcare, behavioral health, nonprofit services, community programs, disability services, or social-service organizations may already have many of the skills needed for this role.
We're looking for someone who understands that effective care coordination is built on trust, communication, and making sure people receive the support they need when they need it.
