Care Coordinator

Job Summary
We are looking for a Care Coordinator to support individuals by managing their daily care needs and ensuring smooth communication between them and their service providers. This role involves organizing schedules, assisting with daily tasks, and helping individuals access the resources they need. Responsibilities

• Arrange and oversee daily care plans.
• Communicate with individuals, staff, and service providers.
• Assist with scheduling and appointment reminders.
• Provide guidance on available support resources.
• Ensure individuals receive the care and services they need.
• Maintain accurate records.
• Work collaboratively with a team to enhance overall support.

Requirements

• High school diploma
• Experience in coordinating services or support roles preferred.
• Strong communication and organizational skills.
• Ability to multitask and problem-solve effectively.
• Passion for helping others and improving their well-being.
• Ability to work independently as well as part of a team in a fast-paced environment.

Job Type: Full-time

Pay: From $21.00 per hour
