Care Coordinator

Duties

• Develop, implement, and regularly review individualized care plans tailored to each resident’s needs and preferences
• Coordinate with healthcare professionals, families, and care staff to ensure comprehensive support for residents
• Supervise care staff, providing guidance, training, and leadership to uphold quality standards and best practices
• Manage daily schedules to ensure timely delivery of care services and activities
• Oversee documentation related to resident care, including progress notes, incident reports, and compliance records
• Facilitate effective communication among team members, residents, and families to promote transparency and trust
• Ensure adherence to health and safety regulations within the care environment

Skills

• Strong driving skills with a valid license to facilitate resident outings or appointments when needed
• Experience working in a care home or senior care setting with a focus on person-centered support
• Supervising experience demonstrating leadership capabilities in managing teams of caregivers or healthcare assistants
• Excellent communication skills to effectively liaise with residents, families, healthcare professionals, and team members
• Knowledge of creating and updating care plans aligned with regulatory standards and individual needs
• Proficiency in IT systems used for documentation, scheduling, and communication within healthcare settings
• Ability to multitask efficiently while maintaining attention to detail in a fast-paced environment

Pay: £25,420.00-£44,020.88 per year
