- Working with health, social care, voluntary and community partners to ensure joined-up, efficient and person-centred care planning.
- Maintaining accurate records, signposting to appropriate resources, supporting multidisciplinary meetings and liaising with patients, families and carers by phone, in clinics or during home visits as required.
Who we’re looking for:
- Someone with excellent communication and relationship-building skills, able to work positively with patients, carers, professionals and community partners.
- A well-organised and resilient team member who can prioritise a varied workload, work under pressure and remain calm when dealing with complex or sensitive situations.
- Someone confident using digital systems and Microsoft packages, with an awareness of clinical or care systems such as SystmOne being desirable.
- A person who shares our commitment to confidentiality, safeguarding, inclusion and improving outcomes for people receiving care and support.
