Job Overview

We are seeking a dedicated Care Coordinator to join our team. As a Care Coordinator, you will coordinate patient schedules, cover open shifts, participate in rotating on-call schedule, and ensure compliance with regulations while delivering exceptional customer service.

The ideal candidate will possess strong organizational skills, homecare scheduling experience, and a compassionate approach to patient care. Proficiency in Microsoft Office applications including Word, Excel, and Outlook for documentation and communication purposes is required. Knowledge of health regulation policies, health insurance procedures, and electronic health records (EHR/EHR) management systems such as EMR/EHR platforms is a plus.

Pay: $18.00 - $23.00 per hour
