Licensed Practical Nurse (LPN) – Care Coordination
Full-Time | Hybrid (Remote & Field-Based)
Company: Integrative Health Care Services of Indiana (IHCS)
Department: Care Coordination
Location: Assigned service region in Indiana
Reports To: Care Coordination Leadership
Position Overview
At Integrative Health Care Services of Indiana (IHCS), our mission is to improve the quality of life for seniors and individuals with disabilities by providing compassionate, coordinated, and person-centered care.
The Licensed Practical Nurse (LPN) – Care Coordination serves as an advocate, educator, and trusted partner for clients by coordinating services, identifying barriers to care, and facilitating access to appropriate healthcare resources and community-based supports. This position combines the qualifications of a licensed practical nurse with the nonclinical care coordination responsibilities necessary to support clients in maintaining their independence, dignity, and quality of life.
The LPN works collaboratively with clients, families, healthcare providers, waiver case managers, caregivers, and community partners to facilitate appropriate services and support continuity of care. Responsibilities include client-facing visits, care coordination, communication, documentation, service follow-up, and compliance with applicable program requirements.
This is a nonclinical care coordination position. IHCS does not provide direct clinical treatment through this role. The LPN will use their professional knowledge to support service coordination, recognize concerns that require referral or escalation, and facilitate communication with appropriate healthcare professionals. The position does not authorize the employee to perform activities outside IHCS's approved service scope or applicable regulatory requirements.
Success in this role requires compassion, accountability, strong communication, attention to detail, sound judgment, and the ability to build trusting client relationships while managing an assigned caseload in a hybrid work environment.

- Collaborating with healthcare providers, waiver case managers, caregivers, community agencies, and internal team members to support continuity of care.
- Coordinating appointments, referrals, service arrangements, and follow-up communications as appropriate to the scope of IHCS services.
- Providing clients and families with information about available resources, authorized services, and applicable care coordination plans.
- Monitoring service coordination needs and following up on outstanding referrals, identified concerns, and agreed-upon actions.
- Advocating for clients while promoting independence, dignity, informed decision-making, and person-centered service delivery.

- Performing additional duties as assigned that are consistent with the position, the employee's qualifications, and applicable program requirements.
Client Monitoring and Escalation
- Maintain professional, consistent communication with assigned clients and authorized members of their support networks.

- Promptly communicate significant concerns through established IHCS reporting and escalation procedures.
- Coordinate with appropriate healthcare professionals when clients report clinical concerns or request medical advice.
- Maintain professional boundaries and avoid independently making clinical decisions, providing clinical treatment, or representing coordination activities as nursing services.
Documentation and Regulatory Compliance
- Maintain complete, accurate, objective, and timely client records reflecting contacts, visits, coordination activities, referrals, follow-up efforts, and outcomes.
- Complete required documentation, reports, and other records within established deadlines.
- Ensure documentation meets IHCS standards and applicable Indiana Medicaid, FSSA, and program-specific requirements.
- Maintain confidentiality and comply with HIPAA, applicable privacy requirements, and IHCS policies.
- Participate in quality assurance reviews, documentation audits, required training, and performance improvement activities.
- Address identified documentation deficiencies promptly and follow established corrective procedures.
- Ensure all services and activities performed remain within IHCS's approved service scope and applicable program limitations.
Scope of Practice and FSSA Service Requirements
