What they need

Care Coordinator – Domiciliary Care, Live-in Care, Domestic Cleaning & Supported Living

Job Summary

Emzenith is seeking an organised, proactive and compassionate Care Coordinator to coordinate our domiciliary care, live-in care, domestic cleaning and supported living services.

You will help ensure clients receive reliable, person-centred services that respect their choices, dignity and independence. Working closely with management, you will coordinate staff rotas, support care and support planning, monitor service quality and maintain effective communication with clients, families and professionals.

This role also includes actively identifying new business opportunities, developing referral relationships and following up enquiries to help grow Emzenith’s client base. It suits someone who combines strong care coordination skills with the confidence to promote services and build professional relationships.

Key Responsibilities

Service Coordination and Quality

  • Coordinate daily domiciliary care visits, live-in care arrangements, domestic cleaning bookings and supported living staffing.
  • Prepare and maintain staff rotas, matching staff skills, availability and experience to clients’ assessed needs and preferences.
  • Arrange cover for sickness, annual leave and emergencies, maintaining continuity and communicating changes promptly.
  • Coordinate initial assessments, care and support plans, risk assessments and service reviews with the manager and relevant professionals.
  • For domestic cleaning clients, agree task schedules, access arrangements and service expectations.
  • Support live-in care placements, including staff introductions, handovers, breaks and replacement cover.
  • Promote choice, independence and person-centred support for people living in supported accommodation.
  • Monitor visit attendance, punctuality, missed calls and service delivery, escalating concerns promptly.
  • Maintain accurate records of enquiries, assessments, rotas, reviews, incidents and client communications.

Staff Support and Communication

  • Support the recruitment, induction and ongoing supervision of care workers, support workers, live-in carers and domestic cleaners.
  • Arrange spot checks and gather client feedback to identify improvements and staff development needs.
  • Liaise with clients, relatives, representatives, social workers, commissioners and healthcare professionals.
  • Respond professionally to concerns and complaints, referring matters to management where appropriate.
  • Recognise and promptly report safeguarding concerns, incidents and changes in clients’ needs.

Client Acquisition, Referrals and Business Development

  • Actively seek suitable new clients and referral opportunities across Emzenith’s four service areas.
  • Build and maintain relationships with social care teams, discharge teams, direct payment support organisations, housing providers, community organisations and other potential referral partners.
  • Contact prospective referral partners by telephone, email and agreed visits to introduce Emzenith’s services.
  • Respond promptly to enquiries, explain service options and coordinate assessments and quotations with management.
  • Follow up enquiries and referrals through to service commencement, ensuring packages match assessed needs, staffing capacity and agreed terms.
  • Promote current service availability and supported living vacancies using accurate, approved information.
  • Attend relevant networking events, community meetings and provider forums.
  • Maintain an outreach and referral tracker, recording contacts, follow-up actions, enquiries and outcomes.
  • Work towards agreed targets for outreach activity, referral relationships and new service starts while maintaining quality and suitability.

Essential Experience and Skills

  • Experience in care coordination, senior care work, field care supervision or a comparable community care role.
  • Understanding of domiciliary care and/or supported living, with a willingness to develop knowledge across all four services.
  • Experience preparing rotas and responding effectively to staffing changes.
  • Strong organisational skills and the ability to prioritise competing demands.
  • Clear, professional communication and the confidence to contact potential clients and referral partners.
  • A proactive approach to following up enquiries and developing professional relationships.
  • Understanding of person-centred support, safeguarding, confidentiality and accurate record-keeping.
  • Confidence using scheduling software, email, spreadsheets and electronic care records.
  • Ability to travel within the service area for assessments, staff checks and professional meetings.

Desirable

  • Previous experience coordinating live-in care or domestic cleaning services.
  • Experience generating care enquiries, securing referrals or supporting business growth.
  • An established network of local health, social care, housing or community contacts.
  • Level 3 qualification in Health and Social Care, or a relevant equivalent.
  • Full driving licence and access to a vehicle for work-related travel.

Working Arrangements

This role combines office-based coordination with community visits and referral development. Any on-call, evening or weekend responsibilities will be discussed and agreed during recruitment.

How to Apply

Please submit your CV and a brief supporting statement outlining your care coordination experience and how you would help Emzenith attract clients and develop referral relationships.
