Pay: $20–$22 per hour
Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.; some weekend coverage may be required.

Connected Care is seeking a dependable, organized Care Coordinator to coordinate client services, support caregivers, and keep daily schedules running smoothly.

This position requires clear communication, sound judgment, and consistent follow-through. You should be comfortable responding to schedule changes, finding coverage for open shifts, and keeping clients, families, and caregivers informed.

Responsibilities

• Coordinate caregiver schedules based on client needs and staff availability.
• Respond promptly to call-offs, scheduling changes, and open shifts.
• Communicate schedule updates and service concerns to clients, families, caregivers, and leadership.
• Help match caregivers with clients based on care needs, experience, and compatibility.
• Maintain accurate schedules, contact notes, and service records.
• Support caregiver onboarding and communicate assignment expectations.
• Identify concerns, notify the appropriate team member, and follow through.
• Protect confidential information and treat clients, families, and coworkers with respect.

Qualifications

• Experience in care coordination, home care scheduling, staffing, or a related service role preferred.
• Strong organization and attention to detail.
• A calm, professional approach to urgent changes and difficult conversations.
• Ability to manage competing priorities and communicate promptly.
• Comfort using scheduling software, email, and basic office technology.
• Dependability, accountability, and commitment to consistent client care.

Pay: $20.00 - $22.00 per hour

Expected hours: 40.0 per week
