Care Coordinator (EHS) [Ref#YTMDHC]

Key Responsibilities

• Lead and supervise health risk assessments for elderly clients;
• Mentor junior nursing staff and serve as a clinical resource;
• Develop individualized care plans and monitor client progress with multidisciplinary approach;
• Perform advanced assessments for elderly clients with multiple conditions;
• Identify urgent health risks and prioritize follow-ups;
• Design health promotion programs and educational materials;
• Participate in service planning and quality improvement initiatives;
• Maintain accurate documentation and ensure compliance with clinical guidelines.

Entry Requirements

• Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong;
• Degree in Nursing or equivalent;
• Minimum 5 years of working experience in related nursing practice as Registered Nurse;
• Recognised Post-registration training in
  • Post-registration certificate course in gerontology; or
  • Fellow member of Hong Kong College of Gerontology Nursing; or
  • Recognition under voluntary scheme on advanced / specialized nursing practice in gerontology; or
  • Recognized postgraduate certificate/diploma in gerontology nursing
• Experience in care coordination or case management roles;
• Leadership or supervisory experience;
• Experience in health education and program development;
• Track record of quality improvement initiatives.
