Schedule: Full-Time

About the Role

The Care Coordinator Extender supports IMBH’s Behavioral Health Transitions of Care team by completing the administrative and coordination work needed to prepare members for clinical review and ongoing care management. The role works closely with Embedded Care Coordinators and Embedded Clinical Care Coordinators, with a primary focus on preparing cases in TruCare, gathering records, coordinating services, and tracking required follow-up.

The Care Coordinator Extender must also be comfortable providing direct member outreach and enrollment support when needed. This position does not make clinical determinations or independently perform clinical assessments.

What You'll Do

  • Prepare new and transitioning member cases in TruCare so clinicians have complete, organized information available for assessment and care planning.
  • Enter available information from admission notifications, discharge documentation, assessments, and other records into the appropriate TruCare fields.
  • Gather and organize relevant member information, including demographics, diagnoses, utilization history, facility information, discharge details, providers, scheduled follow-up, and identified service needs.
  • Identify missing or incomplete information and request records from hospitals, residential treatment centers, outpatient providers, health plan teams, and other partners.
  • Maintain clinician worklists and track members through referral, outreach, enrollment, discharge, and post-discharge follow-up.
  • Monitor time-sensitive activities, including discharge dates, outreach attempts, follow-up appointments, referrals, and unresolved coordination needs.
  • Support appointment scheduling, referral follow-through, transportation coordination, record requests, and connections to healthcare and community resources.
  • Communicate administrative updates, barriers, and outstanding needs to the assigned clinician so they can prioritize clinical follow-up.
  • Document outreach, coordination activities, status updates, and next steps accurately and promptly in TruCare and other required systems.
  • Provide direct member outreach and enrollment support when assigned, including explaining the program, confirming member information, completing non-clinical enrollment steps, and helping members connect with their care coordinator.
  • Use approved scripts, workflows, and escalation procedures during member outreach.
  • Immediately escalate urgent needs, safety concerns, or information requiring clinical review to the appropriate clinician or supervisor.
  • Communicate professionally with members, caregivers, facilities, providers, health plan staff, and community organizations.
  • Protect member confidentiality and complete all work in accordance with HIPAA, contractual requirements, regulatory standards, and IMBH policies.
  • Participate in team meetings, training, quality reviews, and workflow improvement activities.
  • Perform other duties as assigned.

What You'll Bring

  • Strong organization and attention to detail, especially when preparing cases and entering information into care-management systems.
  • The ability to manage multiple members, deadlines, and follow-up needs without losing track of important details.
  • A proactive approach to identifying missing information, following up on open items, and keeping clinicians informed.
  • Clear, professional communication with clinicians, members, caregivers, providers, facilities, and community partners.
  • Comfort working primarily behind the scenes while also being ready to conduct member outreach and support enrollment when needed.
  • Good judgment about when an issue can be handled administratively and when it needs to be escalated for clinical review.
  • Flexibility and willingness to learn new systems, workflows, and program requirements.

Requirements

  • High school diploma or GED.
  • At least one to two years of experience in healthcare, behavioral health, social services, care coordination, administrative support, customer service, or a related setting.
  • Ability to review records, accurately enter information, and maintain timely documentation.
  • Proficiency with email, spreadsheets, electronic documentation systems, and virtual meeting platforms.
  • Ability to communicate professionally by telephone and in writing.
  • Ability to maintain member confidentiality and follow HIPAA and organizational requirements.
  • Ability to work independently and remain responsive to a remote team.
  • Must reside in Indiana and be available during established Indiana program hours.
  • A private remote workspace appropriate for handling confidential member information.

What Will Make You Stand Out

  • Experience supporting licensed clinicians, care managers, social workers, counselors, nurses, or other healthcare professionals.
  • Experience preparing cases for clinical review by gathering records, organizing member information, identifying missing documentation, and maintaining worklists.
  • Experience with TruCare or another electronic health record, care-management, utilization-management, or case-management platform.
  • Familiarity with behavioral health transitions of care, discharge planning, complex care management, or post-discharge follow-up.
  • Experience coordinating appointments, referrals, transportation, medical records, and community resources.
  • Experience conducting member outreach, explaining healthcare or social-service programs, and supporting enrollment.
  • Familiarity with Indiana Medicaid, behavioral health providers, treatment facilities, and community resources.
  • Comfort communicating with members experiencing behavioral health conditions, substance use concerns, housing instability, or other complex needs.
  • An associate or bachelor’s degree in healthcare administration, behavioral health, social services, human services, or a related field.
