Care Coordinator/ Registered Nurse (CC/RN) (Ref: PHS/DHCE/CC/RN/JDB)

Responsibilities:

• Assist the Senior Care Coordinator to lead nursing team
• Plan and deliver health promotion, education and chronic disease management programs
• Act as a case manager of disease /complication screening, coordinate referrals to & from other medical and allied health disciplines
• Provide direct Nursing service, health assessment screening, health coaching, education and other related services
• Assist supervisor to actively engage service partners to develop primary health care resource hub

Requirements:

• Degree in Nursing or equivalent; holder of valid RN practicing certificate by NCHK; completed post-registration course in Primary Health Care or Women Health or Elderly Health or other related fields preferable
• Possesses 6 years or above of clinical experience as RN
• Experience in Risk Assessment Management Program (RAMP) or Public Private Partnership (PPP) Program is preferable
• Proactive, mature and independent; possess good computer literacy
