Care Manager

About the Role: As a Care Manager, you will play a key leadership role in ensuring the delivery of safe, high-quality, resident-centred care in our lovely home. Reporting to the Residential Service Manager, you will guide and support our care team, ensuring compliance with the Aged Care Quality Standards and Apollo Care’s model of care.
 
Key Responsibilities:

• Lead and support the care team to deliver holistic, person-centred care.
• Oversee care planning, clinical governance, and resident assessments.
• Ensure compliance with regulatory requirements, including the Aged Care Act 1997.
• Promote resident choice, family engagement, and a strong sense of home.
• Drive continuous improvement in quality, safety, and resident experience.

Nanyima Community: To offer a more personal living experience, our 51-bed community is divided into smaller households and has plenty of outdoor spaces to enjoy. Nanyima is just a 30-minute drive from Mackay, nestled amongst sugarcane plantations and sugar mills, and is close to beautiful rainforests and national parks.

About You:

• Current registration with AHPRA as a Registered Nurse
• Minimum 5 years’ experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 2 years in aged care)
• Strong leadership and team management skills
• Excellent knowledge of the Strengthened Aged Care Quality Standards and clinical governance.
• A genuine commitment to supporting residents to live their way with dignity and grace
• Strong verbal and written communication skills
• Compassionate and Empathetic nature
