Care Manager

Rehabilitation Support Services’ Care Management Program currently has an opening for a Health Home Plus/AOT Care Manager serving Delaware County.

The schedule for this position is Monday through Friday, 8:00 AM–4:30 PM, with some flexibility.

The primary responsibility of the Care Manager is to provide intensive Health Home services to a caseload of approximately 12 individuals, with the goal of reducing hospitalizations through engagement with appropriate community-based resources and services, as well as behavioral and physical health care. The Care Manager will work closely with community providers to ensure the appropriate delivery of services to meet each participant’s individual needs.

Responsibilities include:

• Conducting assessments to identify client needs and developing individualized care plans.
• Providing intensive care management services to Health Home Plus (HH+) and Assisted Outpatient Treatment (AOT) clients.
• Meeting with AOT clients in person weekly and HH+ clients in person twice per month.
• Monitoring and ensuring adherence to AOT court orders.
• Maintaining frequent communication and collaboration with community providers.
• Submitting weekly reports to the County.
• Attending County meetings and participating in care coordination activities.
• Managing multiple ongoing tasks while effectively prioritizing responsibilities.

The ideal candidate is highly organized, dependable, and able to work independently. Strong written, verbal, interpersonal, and computer skills are essential, along with the ability to effectively coordinate services across multiple providers and systems.

Minimum Qualifications:

• Bachelor’s Degree in Human Services or a related field; OR
• CASAC with at least 2 years of experience in the Human Services field.

Pay: $25.16 - $27.30 per hour
