What you'll do

As a care manager at Lighthouse Care, you'll serve as the primary point of contact for a caseload of members enrolled in the Health Home program. You'll work across health, behavioral health, and social service systems to help members achieve their goals and improve their quality of life.

• Conduct comprehensive assessments and develop individualized care plans in collaboration with members and their support networks
• Coordinate care across primary care, behavioral health, specialist, and community service providers
• Conduct regular outreach and in-person visits to engage and support members
• Use our care management platform to document member interactions, track progress, and manage tasks efficiently
• Connect members to housing, benefits, transportation, food access, and other social determinants of health services
• Participate in care team meetings, supervision, and ongoing trainings
• Meet documentation and productivity standards in accordance with Health Home requirements

What we're looking for

Required

• Bachelor's degree in social work, psychology, human services, or related field (or equivalent experience)
• 1+ year of experience in care management, case management, or social services preferred
• Familiarity with Medicaid populations and health home or similar programs
• Strong organizational and time management skills
• Comfort using technology — EHRs, care platforms, and digital tools
• Valid driver's license and reliable transportation

Nice to have

• Experience in a Health Home CMA or managed care setting
• Bilingual Spanish/English or other language a plus
• LMSW or LCSW (or working toward licensure)
• Experience with specific care management platforms (e.g., Foothold, Netsmart etc.)
• Knowledge of local community resources and providers
