This role suits someone organised and proactive who stays calm under pressure and cares about the people we serve and the staff who serve them.

What you’ll do

Client care

  • Act as the primary contact for clients and families, and handle questions and concerns with empathy and professionalism
  • Conduct assessments and develop and regularly review individualised care plans
  • Check in with clients routinely (in person, by phone or virtually) and adjust care as needs change
  • Coordinate with nurses, therapists and other healthcare professionals
  • Document and resolve client concerns, complaints and incidents promptly

Caregiver support and supervision

  • Guide and support a team of caregivers, and give fair, constructive feedback
  • Review and approve caregiver notes and documentation
  • Build a positive work environment that keeps good caregivers with us

Scheduling

  • Match each client’s needs with caregiver skills and availability
  • Handle call-offs, schedule changes and urgent staffing needs quickly

Authorisations and compliance

  • Verify and track insurance authorisations for skilled and non-skilled services
  • Manage re-authorisations ahead of time so care and billing aren’t interrupted
  • Conduct quality assurance checks, client satisfaction surveys and caregiver audits
  • Share on-call duties and use sound judgment to resolve issues

EVV, billing and payroll

  • Train caregivers on EVV clock-in/clock-out in Kantime
  • Verify and approve EVV visits daily, and resolve missed punches and discrepancies
  • Work with billing to make sure visit data turns into accurate invoices
  • Review hours in Kantime for payroll and resolve discrepancies

Incident reporting and administration

  • Serve as the point of contact for incidents (falls, medication errors, employee injuries) and document them in Kantime and the state EIM system
  • Keep client and employee records and schedules current in Kantime
  • Generate reports on care, staff hours, PTO and authorisation status
  • Attend the daily morning huddle and report on incidents and activities

What you’ll bring

Required

  • 2+ years in care management, scheduling or client services in a home care or home health setting
  • Working knowledge of insurance verification and authorisation for skilled and non-skilled services
  • Experience managing an EVV system and keeping staff compliant
  • Understanding of healthcare billing and payroll processes
  • Strong written and verbal communication skills
  • Excellent organisation, time management and problem-solving skills
  • Comfort with computers, EMR systems and scheduling software

Preferred

  • Kantime experience (strongly preferred)
  • Experience with Pennsylvania Medicaid / Community HealthChoices programs

Personal qualities

  • Able to work independently and juggle priorities in a fast-paced environment
  • Patient and empathetic, with a real commitment to serving older adults and people with disabilities
