Care Navigator

Join a Mission-Driven Home Care Team
Rapha Homecare is seeking a compassionate, organized, and client-focused Care Navigator / Intake Specialist to support the growth of our home care services in Sacramento and the surrounding service areas. This field-based role serves as the primary point of contact for new clients and their families, conducting in-home assessments, creating care plans, coordinating service starts, and performing ongoing reassessments to ensure exceptional client care.

Duties & Responsibilities:

• Conduct in-home assessments for prospective and newly admitted clients.
• Evaluate client care needs, home safety concerns, and support requirements.
• Develop individualized service plans based on assessment findings.
• Complete routine and change-of-condition reassessment visits.
• Educate clients and families about available services and care options.
• Coordinate service initiation with scheduling and operations teams.
• Support caregiver-client matching based on client needs and preferences.
• Maintain timely and accurate documentation within the SwyftOps platform or other Agency Management System, including assessments, reassessments, service plans, visit notes, and client communications.
• Monitor client satisfaction and identify opportunities to improve delivery of service.
• Serve as a liaison between clients, families, caregivers, referral sources, and agency staff.
• Participate in quality assurance and compliance activities.

Experience & Skills

• High school diploma or equivalent.
• At least 2 years of experience in home care, healthcare, case management, client services, social services, or related field.
• Strong interpersonal and customer service skills.
• Excellent verbal and written communication skills.
• Valid California driver's license, reliable transportation, and auto insurance.
• Ability to travel throughout the service area for client visits.
• Basic computer proficiency and ability to document electronically.
• Bachelor’s degree in healthcare administration, Social Work, Gerontology, Human Services, Nursing, or related field a plus
• Experience conducting client assessments and care planning.
• Knowledge of California home care regulations.
• Bilingual English/Spanish or other languages.
