Help families find the right care—and keep every next step moving forward.
Nye Health Services is seeking an organized, compassionate, and proactive Care Navigator to support prospective residents and families as they explore our services. If you enjoy connecting with people, managing details, and making sure follow-up happens, this could be a great fit.
This role is part of our centralized sales support function, based through our Home Office in Fremont, with the opportunity for candidates to work from Fremont or Norfolk.
About the Role
As a Care Navigator, you will often be one of the first people a prospective resident or family connects with. You will listen to their needs, answer initial questions, explain available services, and connect them with the appropriate community and team.
This is a highly task-oriented position that combines relationship building with consistent follow-through. Success requires keeping inquiries organized, documenting conversations accurately, scheduling next steps, and ensuring families receive timely support throughout their search.
What You’ll Do
- Respond promptly to new inquiries by phone, email, and other assigned channels, working toward a 30-minute response goal during business hours.
- Connect with prospective residents and families early in their journey to understand their needs, preferences, timeline, and questions.
- Develop a strong understanding of Nye’s services, including independent living, assisted living, skilled nursing, rehabilitation, and home health, to help families explore appropriate options.
- Coordinate tours and connect inquiries with community sales, clinical, and operational teams for further assessment and next steps.
- Maintain accurate CRM records, including meaningful conversation notes, lead status, scheduled follow-up, and a clear next step for every active inquiry.
- Provide consistent follow-up that helps families feel informed and supported while moving toward a tour or another appropriate next step.
- Provide backup support for other Nye communities during team member absences or periods of increased inquiry volume.
- Collaborate with sales, marketing, clinical, and operational leaders to support smooth handoffs and a responsive customer experience.
What You Bring
- Strong organization, time management, and task prioritization skills.
- A dependable follow-up approach and close attention to detail.
- Clear, professional communication and a warm, helpful phone presence.
- The ability to ask thoughtful questions, listen carefully, and explain options in an understandable way.
- Comfort managing multiple inquiries and deadlines while keeping accurate records.
- Confidence using computers, learning CRM systems, and working with digital task lists and reports.
- The ability to work independently while staying connected with teams across multiple locations.
- A service-oriented mindset and respect for the sensitive decisions families may be facing.
Work Environment
This is primarily an office-based position involving frequent telephone communication, computer use, CRM documentation, and coordination with community teams. Candidates may be located in Fremont or Norfolk and will work within Nye’s centralized sales support structure.
