The position provides structured outreach, caring contacts, warm handoffs, service linkage and barrier resolution following acute level of care discharge. The Care Navigator's primary responsibility is timely post-CSU engagement and navigation, to ongoing behavioral health, substance use, primary care, and community-based services; ensuring that the individual has a meaningful connection to ongoing care and does not become lost during the transition between systems by facilitating appropriate referrals and warm handoffs and helping ensure continuity while responsibility transitions from the acute care setting to the receiving care team.
