What they need

About the role

An exciting opportunity is available for an experienced Care Partner to join Odyssey Lifestyle Care Communities, supporting residents across home care, ageing in place and assisted living.

You’ll deliver high-quality nursing care, including clinical assessments, referrals, medication administration and wound management. Supporting older people through the Support at Home program, you’ll provide comprehensive assessments, ongoing care planning, monitoring, service coordination and education, with a strong focus on wellness, reablement and culturally safe care.

Working closely with residents, their chosen supporters and external providers, you’ll tailor services to meet clinical and non-clinical needs while building trusted relationships. The role combines clinical judgement with care coordination, education and advocacy. You’ll also review individual budgets, support accurate billing and help residents make effective use of their available funding.

This role is based at our Chevron Island site, with a requirement to work at our Robina site when needed.

Key Responsibilities

  • Assess residents’ needs: Conduct initial and ongoing assessments of physical, emotional and social needs, identify risks, and tailor care with residents and their chosen supporters.

  • Develop care plans: Develop, review and update person-centred care plans that reflect residents’ goals, preferences, changing needs and available Support at Home funding.

  • Coordinate services: Organise in-home supports and liaise with internal teams and external providers to ensure timely, coordinated and quality care.

  • Advocate and educate: Help residents understand their funding, choices and rights, and advocate for access to appropriate services.

  • Monitor budgets: Review individual budgets, support accurate billing and guide residents on the effective use of their funds.

  • Build relationships: Develop strong relationships with residents, families, carers and providers to support smooth transitions and continuity of care.

  • Maintain compliance and safety: Keep accurate documentation, uphold Support at Home requirements and aged care standards, and proactively identify, manage and escalate risks.

Skills and Qualifications

  • Demonstrated experience in care planning, case management and clinical assessment within aged care, community care or healthcare settings.
  • A minimum of one year’s relevant care planning and case management experience, with two years preferred.
  • A sound understanding of the Support at Home program, strengthened Aged Care Quality Standards and Statement of Rights.
  • Strong communication, organisation and problem-solving skills.
  • Ability to interpret clinical guidance, recognise changes in residents’ needs and escalate concerns appropriately.
  • Current registration as a Registered Nurse with AHPRA.

We’re an innovative lifestyle resort with warmth, independence and clinical excellence at our core.

  • Enjoy exceptional working conditions, with thoughtfully designed facilities, natural light, high-end amenities and purpose-built spaces that support quality care delivery.
