This is ‘Care Management’ you can feel proud of

At Carer's Network, a Care Partner is never just a name at the end of a phone. You are the trusted person who listens, understands what matters, and brings the right support together. You will see the difference your decisions make - in a client's confidence, a family's reassurance, and an older person's ability to keep living life their way.

This is a hands-on, relationship-led role for someone who enjoys variety, takes ownership, and knows that great care starts with listening. You will have the autonomy to solve problems, the backing of a supportive team and the opportunity to help shape quality care across Bendigo and surrounding communities.

Why this role is worth making your move

· Purpose you can see. Your work directly helps older people remain safe, connected, and independent at home.

· Relationships that matter. Know your clients, earn their trust, and walk alongside them as their needs and goals change.

· Real ownership. Manage your portfolio, make informed decisions, and turn care plans into practical outcomes.

· A voice that is heard. Join a family-owned, growing organisation where ideas, initiative and local knowledge are valued.

· Room to grow. Access supported learning, ongoing industry training, and employee wellbeing initiatives.

This Could Be the Move Your Career Has Been Waiting For!

What you will own

· Client relationships. Be the primary point of contact for clients and families, from referral and assessment through to ongoing reviews.

· Care planning. Conduct comprehensive in-home assessments and build personalised plans around each client's strengths, priorities, and goals.

· Budgets and agreements. Develop individual budgets, manage service agreements, and monitor package utilisation in line with Support at Home requirements.

· Service coordination. Partner with Scheduling, care workers, allied health professionals, and other stakeholders to deliver seamless support.

· Quality and compliance. Maintain accurate records and ensure services meet the Aged Care Quality Standards and regulatory requirements.

· Better ways of working. Spot opportunities to strengthen the client experience, improve service delivery and support sustainable growth.

You will thrive here if you

· Lead with people. You are warm, perceptive, and genuinely person-centred, with the confidence to hold clear and respectful conversations.

· Know community care. You bring at least one year of relevant case management experience in home-based aged care or community services; deeper experience is highly regarded.

· Understand the detail. You can manage budgets, priorities, documentation, and compliance without losing sight of the person behind the plan.

· Stay calm and find a way forward. You are organised, digitally capable and comfortable making decisions in a changing environment.

· Bring relevant training. A diploma or tertiary qualification in aged care, community services, disability, social work, nursing, allied health, or a related field is highly regarded. A nursing background is desirable.

· Can travel locally. You hold a valid Victorian Driver Licence.

· NDIS Worker Screening Check. You have or are willing to obtain the required NDISWSC clearance.

Bring your experience somewhere it will matter

Carer's Network is a local, family-owned provider supporting older Australians and NDIS participants to live independently at home. We are big enough to provide trusted systems and support, yet small enough for people, relationships, and good ideas to remain at the centre of what we do.

Ready to make your move? Submit your resume and a brief cover letter explaining why you would be a brilliant Care Partner with Carer's Network.
