Care Planner

Contract Stoke-on-Trent, Staffordshire

About The Role As a Care Planner you'll work closely with people who use our services, their families and our commissioning partners to develop support plans that are genuinely person-centred, measurable and compliant. This is a contract role suited to an experienced professional who wants to contribute to a new service model from the ground up.

Responsibilities

• Carry out assessments and develop detailed, person-centred support plans that genuinely reflect the individual's goals, routines and preferences
• Work with the person, their family, social workers and commissioners throughout the planning process
• Ensure plans meet relevant care standards and capture meaningful, measurable outcomes
• Review and update support plans at regular intervals, responding to changes in need
• Maintain accurate and compliant documentation in line with regulatory requirements
• Support the organisation to develop consistent, high-quality planning processes

What we're looking for

• Demonstrable experience in care planning, case management or a related assessment role
• Strong working knowledge of person-centred planning approaches and relevant legislation
• Experience working with local authority commissioners and multi-agency teams
• Excellent written skills — the ability to write plans that are clear, person-centred and professionally presented
• NVQ/QCF Level 4 or above, or equivalent professional qualification (desirable)
• Enhanced DBS check required
