Support Worker – Supported Living Services

Main Duties

• Provide personal care and daily living support.
• Support service users with their routines, independence and wellbeing.
• Assist with community activities, appointments and social engagement.
• Follow care plans, risk assessments and behaviour support plans.
• Support individuals who may display behaviours that challenge.
• Use appropriate positive behaviour support and de-escalation techniques.
• Provide medication support or reminders, subject to training and competency.
• Monitor and report changes in service users’ health, wellbeing or behaviour.
• Complete accurate and timely digital care records.
• Participate in clear and effective shift handovers.
• Report incidents, safeguarding concerns and identified risks promptly.
• Communicate professionally with service users, families, colleagues and management.
• Maintain dignity, privacy and confidentiality.
• Follow safeguarding, infection control, health and safety and company procedures.
• Attend required training, supervision and team meetings.

Essential Requirements

Applicants must have:

• Between 1–3 years of relevant care experience.
• Experience in Supported Living, mental health, learning disabilities, autism or complex care.
• Experience supporting individuals with behaviours that may challenge.
• The ability to follow care plans, risk assessments and behaviour support plans accurately.
• A good understanding of safeguarding and person-centred support.
• Good written and spoken English.
• Basic IT skills for completing digital care records.
• A full UK driving licence.
• The legal right to work in the UK.
• The ability to provide two professional referees.

Preferred Training and Experience

• PMVA or equivalent de-escalation and physical intervention training.
• PEG feeding training or practical PEG experience.
• Medication administration training.
