We are seeking compassionate and dedicated Caregivers to provide exceptional support and assistance to individuals in various care settings, including home, assisted living, and long-term care facilities. Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
• Monitor and observe patients for any changes in condition, reporting concerns promptly
• Administer medications and manage care plans according to healthcare provider instructions
• Prepare nutritious meals and assist with meal planning to meet dietary requirements
• Support patients with memory care needs, including dementia and Alzheimer's care
• Operate patient monitoring equipment and record vital signs accurately
• Maintain a clean and safe environment by performing light cleaning duties
• Assist with transfers using equipment such as Hoyer lifts or resident lifts while ensuring safety protocols are followed
• Follow HIPAA guidelines to protect patient confidentiality at all times
• Provide emotional support and companionship to residents or clients, fostering a respectful and caring atmosphere
• Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive care

Skills

• Proven experience in home & community care, assisted living, nursing homes, or long-term care environments
• Knowledge of patient observation, medication administration, and vital signs monitoring
• Certifications such as First Aid, CPR, HIPAA compliance preferred
• Experience working with individuals with developmental disabilities, autism, dementia, or Alzheimer's disease
• Ability to perform heavy lifting and assist with mobility using appropriate equipment like Hoyer lifts
• Strong communication skills for effective interaction with residents, families, and healthcare teams
• Compassionate demeanor with excellent patience and empathy skills
• Ability to follow detailed care plans and adhere to safety protocols
• Flexibility to work various shifts as needed; prior experience in hospice or home health is a plus
