This isn’t a traditional caregiving position. Post Care Partners is a growing company dedicated specifically to supporting clients through the post operative recovery period, and we’re looking for someone who wants to grow with us.

As our Lead Care Partner, you’ll have the opportunity to help shape how care is delivered from the beginning. You’ll work directly with clients while partnering closely with the founder to establish care standards, strengthen processes, mentor future Care Partners, and help create the exceptional client experience we want Post Care Partners to be known for.

Key Responsibilities

• Provide direct, non-medical recovery support in accordance with each client’s individualized care plan.
• Assist with activities of daily living and routine household needs within the approved scope of service.
• Prepare simple meals and assist clients with hydration and everyday recovery routines.
• Provide companionship, mobility assistance, and general safety oversight.
• Provide transportation and accompany clients to approved appointments or errands when scheduled.
• Support clients transitioning home following surgery or hospital discharge.
• Serve as the primary on-the-ground resource for other Care Partners assigned to a client.
• Help orient Care Partners to client preferences, schedules, routines, and established care plans.
• Reinforce Post Care Partners’ service standards and expectations.
• Communicate client concerns, changes in condition, scheduling issues, incidents, or service concerns promptly to leadership.
• Complete required shift documentation and client updates accurately and on time.
• Assist leadership with identifying opportunities to improve the client experience and care delivery.
• Model professionalism, discretion, reliability, and compassionate service at all times.
• Maintain appropriate professional boundaries and protect client confidentiality.
• Recognize situations outside the scope of non-medical care and appropriately escalate them to leadership, the client’s designated contact, or emergency services according to company policy.

Qualifications

• Proven experience in caregiving, companion care or home care experience.
• Reliable transportation and valid drivers license/insurance
• Ability to pass a background check and obtain an Arizona fingerprint clearance card.
• Experience working with individuals that are post surgical patients, have developmental disabilities, Alzheimer’s or dementia and require care
• Familiarity with HIPAA regulations and confidentiality standards in healthcare environments
• Certification in CPR additional certifications such as nursing assistant or home health aide are advantageous
• Excellent communication skills for supervising staff and engaging with residents’ families
• Demonstrated ability to work independently as well as part of a team in fast-paced environments
• This position is a leader, strong worth ethic and compassion is required

Pay: $18.00 - $23.00 per hour
