Home Health Aide

Job Summary:

Responsibilities as a Caregiver:

• Provide one-on-one support to clients in their homes, assisting with daily living activities such as bathing, dressing, and grooming
• Assist with managing medications, incontinence care, and other personal care needs (as needed)
• Monitor and report changes in client condition to the Care Manager and/or supervisor
• Collaborate with the care team to develop and implement individualized care plans
• Maintain accurate and timely records of client care and services provided

Requirements:

• High school diploma or equivalent required
• Must pass a background check and obtain TB test
• Ability to lift up to 50 pounds and stand for extended periods
• Strong communication and interpersonal skills
• Compassionate and patient demeanor
• Ability to work in independently and prioritize tasks accordingly
