Documentation & Communication:

  • Document outcomes related to resident care, behavior, activity, and intake accurately and timely.
  • Communicate changes in health condition, behavior, or care needs to nursing leadership.
  • Share important updates with team members across shifts to ensure continuity of care.
  • Follow all documentation policies and regulatory requirements.

Team Collaboration & Culture:

  • Promote and support a positive work environment.
  • Work closely with nursing, culinary, and life enrichment teams to support the full resident experience.
  • Contribute to a culture of person-centered care and shared accountability.
